QUALIFICATIONS AND REQUIREMENTS:
• A Bachelor's Degree in Psychology, Sociology, or Human Behavioral Science Field.
• One year professional casework experience, if possible.
• An understanding of the dynamics of aging, disability, death and dying and loss and grieving.
• The willingness to protect the right of the client regarding self-determination.
• Position requires the ability to enter a variety of home settings, many of which are not handicap accessible, provide own transportation to clients homes, willingness to enter unsanitary environments, and handle hostile and difficult clients and families.
• The ability to read, write, complete and enter on the computer, complicated forms that are part of Single Entry Point.
• Proficiency in Microsoft Office applications online including, sending and receiving e-mails and navigating the World Wide Web.
• Proficiency in typing, a minimum of 30 WPM, 45 WPM preferred.
• Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers.


ESSENTIAL FUNCTIONS:
• Participates in Organizational Excellence Environment
• Must maintain calm and professional demeanor.
• Assessment of medically fragile clients, including level of functioning, type of disability, family and support systems, program appropriateness; completion of the assessment instrument.
• Target clients for appropriate programs, services and resources.
• Complete initial targeting for clients who apply in person for financial eligibility at RMOLTC office, as requested.
• Obtain medical information from the client's physician(s) in order to initially certify/recertify clients annually.
• Complete ULTC 100.2 and other required forms assessments on the state prescribed client record.
• Document the client's eligibility for programs under Single Entry Point, if appropriate.
• Ability to enter a variety of home settings, many of which are not handicap accessible, provide own transportation to clients homes, willingness to enter unsanitary environments, and handle hostile and difficult clients and families.
• Required to drive to and from client assessments.
• Obtain medical information required from the client's physician in order to complete application process.
• Develop and implement care plans that are cost-effective and provide appropriate services.
• Coordinating and monitoring of service delivery.
• Guiding and assisting clients and families in finding facility placement, if needed.
• Evaluating the effectiveness of services provided in the care plan, as well as cost-effectiveness.
• At least twice yearly assessment of medically fragile clients, including level of functioning, type of disability, family and support systems, program appropriateness; completion of the assessment instrument. Contacts made via telephone and face-to-face interview.
• Act as a client advocate.
